We strive to make your visit and payment to our practice as easy as possible.

Due to the volume of insurance companies and plans, we are unable to provide specific details about your insurance coverage or your network of physicians. Please contact your insurance company with questions about your coverage and access to network physicians.

If your insurance company requires a referral for visits to specialists, you must bring the referral to your appointment in order to see a physician. If you do not have a referral, we ask you to sign a release form acknowledging your appointment was made without a referral.

Co-payments are due at your appointment.  For your convenience, we accept cash, check, money order, Visa, MasterCard and Discover.

If you do not have insurance, full payment is expected at the time of your appointment.
